What do I bring to my procedure?

Don't forget to bring your driver, as much as we like and cherish our patients, we still want to send you home.

The government requires that you have received your Patient Bill of Rights, disclosure of your physician financial interest & ownership in the surgery center and advance directives disclosure. A copy of these forms can be downloaded or read here: Patient Forms

  • Please bring your insurance card and copays as well as a form of identification [Drivers License]
  • Please bring a list of your current medications and dosages.
  • Please DO NOT bring jewelry, other unnecessary valuables such as computers.
  • Wear comfortable clothes.
